Before the work project that required it dried up, we had a quick look 
at PHPSurveyor. http://phpsurveyor.sourceforge.net/index.php

It supports anonymous surveys, and may or may not fulfill the other 
requirements, but it sure looks purty.

Oh, and it only requires PHP & MySQL.
