The PDF isn't a form, so using something like qpdfview (which supports basic
PDF form filling) won't help.  I'm thinking the Print-write-scan/photo route
is probably the easiest approach.  Otherwise there's the option to use
inkscape (or similar) to put text into the PDF without rasterising it (an
approach I've used for some PDF documents in the past), but it might be more
hassle than it's worth in this particular case.
