Sorry, I wasn't clear.  People being what they are, incidents like this
will happen and need to be dealt with.  In Australia as far as I am
aware all conferences have had strong T&C's, and all incidents have been
dealt with well using these those T&C's.  The sort of incident I am
looking for is one where things weren't handled well, forcing LA's hand.
Has this ever happened?

As far as I am aware Australia can stand proud on this issue.  To take
the specific cases Arjen mentioned in his post, the response to the LCA
2007 incident was to eject the perpetrator, and in LCA 2011 the lead
conference organiser apologised.

Finally I've been too negative.  It's probably not clear my comments
only apply to the current proposal.  As I said before I thought the code
Donna put up was exemplary.  It does the minimum that must be done - it
insists that there be no harassment of any sort, and it gives the
organisers permission to act if something goes wrong.  Better yet it
doesn't get too specific and list conditions that might work for one
group but not another.  I guess it could be shorter, but raising the bar
with "respect others" and "be nice" sets a good tone.  Adopting it as a
minimum CoC for any event LCA is involved with seems like prudent course
of action.
