In the event that the President of the United States becomes unfit for office, the Vice President becomes Acting President or The President (if the reason is because of death) for the President's term of office.
If the Vice President is also incapacitated - think something like the Polish Presidential plane crashing - then the Speak of the House takes the role. If the speaker is incapacitated, the President of the Senate takes the role and failing that there is a succession between the Executive Secretaries.
